Article 14 （投票用紙の様式） 

(1)   投票用紙には、審査に付される裁判官の氏名を、中央選挙管理会がくじで定めた順序により、印刷しなければならない。

(2)   投票用紙には、審査に付される各裁判官に対する?の記号を記載する欄を設けなければならない。

Article 68.5

(1)   憲法改正原案について国会において最後の可決があつた場合には、その可決をもつて、国会が日本国憲法第九十六条第一項 に定める日本国憲法 の改正（以下「憲法改正」という。）の発議をし、国民に提案したものとする。この場合において、両議院の議長は、憲法改正の発議をした旨及び発議に係る憲法改正案を官報に公示する。

(1)   憲法改正の発議があつたときは、当該発議に係る憲法改正案の国民に対する広報に関する事務を行うため、国会に、各議院においてその議員の中から選任された同数の委員で組織する国民投票広報協議会を設ける。

Article 56

(2)   投票用紙には、賛成の文字及び反対の文字を印刷しなければならない。

(3)   投票用紙は、別記様式（第六十一条第一項、第二項及び第四項並びに第六十二条の規定による投票の場合にあっては、政令で定める様式）に準じて調製しなければならない。

Referendum onSupreme Court Justices: the CentralElection Commission decides the order in which the names of the Supreme Courtjustices are printed on the ballot.

Referendum onConstitutional amendment: once theconstitutional amendment passes both houses of Parliament, the full text of theproposed amendment is published in the official gazette, along with anexplanation of why the amendment is being proposed. (Article 68.5, Law on Parliament)

The law only specifies that the ballot must have “yes”and “no” printed on it. (Article 56, ActConcerning Procedures to Revise the Constitution)
