?663.  Forms of ballots and envelopes

  (a) Notless than thirty (30) days prior to each election the Supervisor of Electionsshall prepare in such quantities as he deems necessary the following papers:

   1. Absentee ballots similar in form to the official ballotsand containing the forwarding address for the appropriate board of election;provided, however, that the names of candidates may be placed on ballots in theorder in which the candidates filing for a particular office have filed.

   2. Envelopes of sufficient size to contain the ballotsspecified in paragraph 1 of this section; on which shall be printed the words'Official Absentee Ballot' and nothing else.

   4. Envelopes of a sufficient size to contain the ballots andenvelopes specified in paragraphs 1 and 2 above, addressed to the boards ofelections for the several election districts, upon which shall be printed'Enclosed is the ballot of an armed forces absentee voter', and atthe top thereof blank spaces for the name, address and voting place of thesender, with the words 'Name', 'Address', and'Election District' appropriately printed thereon. The Supervisorshall affix air mail, special delivery postage to each such envelope to be sentto an applicant for absentee ballot outside the election district in which heor she is qualified to vote.
