Who decides on the final form of the ballot text?

Constitution of the Republic of Azerbaijan:

Article 134.1 

Referendumballot paper (hereinafter - ballot paper) shall be prepared in accordance withthe rule mentioned in Article 99 of this Code.
134.2 The design of ballot papers is approved bythe Central Election Commission at least 30 days prior the referendum.
